10/13/2023 EnchantaVaughn Approved Behavior: QMHPA arrived to the home of Ms. Broadnax. Upon arrival to the home, Ms. Broadnax appeared to be in a positive mood evident by her facial expressions such as a smile on the face. Ms. Broadnax did not present any symptoms of S/I or H/I at the time of the session. Intervention: QMHPA engaged Ms. Broadnax in a discussion surrounding her mental health. QMHPA openly discussed problems areas that contribute to her daily well-being. QMHPA explored problem-solving techniques to utilize when feeling overwhelmed and having anxieties. QMHPA encouraged positive self-talk for Ms. Broadnax to reframe from negative thinking and acting out negatively. QMHPA engaged Ms. Broadnax in a skills building activity. QMHPA explained to Ms. Broadnax the purpose of this activity was to assist Ms. Broadnax with reducing stress evident by deep breathing to calm herself, and encouraged thought-stopping techniques to replace negative thoughts before acting out with anger and frustrations. Response: Ms. Broadnax shared with QMHPA that her mental health was fine; however, she has been feeling a little depressed. Ms. Broadnax expressed to QMHPA that she doesn’t know where the depression is coming from. During the skills building activity, Ms. Broadnax was understanding of how the skills would assist her with calming herself, and using relaxation techniques to anger negative decisions. Ms. Broadnax expressed to QMHPA that she thinks that she needs to get an updated psychological; however, she knows there is a waiting period due to QMHPA assisting with scheduling one weeks’ ago. QMHPA reiterated that it would better assist her with identifying any concerns that she may have about herself. Towards the end of the session, Ms. Broadnax expressed to QMHPA that she as going to try to use those skills that were taught to her during the session.
